This article discusses Tufts-Delta Health Center, a community health center that has been successful in addressing the social, economic, environmental, and political circumstances that determine the ill health of the community and people it serves. It provides a brief history of the area and population that Tufts-Delta Health Center serves as well as a discussion of how population health could be improved nationwide through the use of community health centers.
