This review focuses on transitions from inpatient to outpatient care, provides an overview of current care transition intervention frameworks and models, and identifies components suited for more effectively managing transitions among persons with severe mental illnesses. It contains a table of curated, relevant model components on care transitions, along with suggestions on how those components could be tailored to a mental healthcare setting.
