This resource investigates the association between community social capital measures (economic connectedness, social cohesion, and civic engagement) and population health outcomes (prevalence of diabetes, hypertension, high cholesterol, kidney disease, and obesity) across zip code tabulation areas (ZCTAs) in all 50 US states.
